WE DO REQUIRE A PARENT OR LEGAL GUARDIAN TO ACCOMPANY THE MINOR RECEIVING A PIERCING.

16-17 WILL NOT REQUIRE A PARENT OR GUARDIAN IF ONLY GETTING EARLOBE PIERCINGS

We will not accept photos or photo copies of ID's or documents.

Minors 4-15

Original or Certified Copy of Birth Certificate for any minor under 16 as well as parents state or federally issued picture ID.

If last names do not match we will also need one document confirming the name change

If you are a legal guardian then you will need to bring guardianship papers from DFAC's or a guardianship decree sign and sealed by a judge.

Minors 16-17 will need to bring

For earlobe piercings

State or Federally Issued ID

For all other piercings

Original or Certified Copy of Birth Certificate

Minors State or Federally Issued ID or School Photo ID

Parents State or Federally Issued ID

RELEASE FORM WILL NEED TO BE FILLED OUT IN PERSON.

PLEASE DO NOT FILL IT OUT IN ADVANCE.

If younger siblings or younger children come with you, please make sure to bring someone for supervision.

Only one parent or guardian is allowed in the piercing area along with the minor.
